[...] Role and contribution of the Somerset West Community Health Centre During the development of the MHN proposal, SWCHC played a key role in supporting the process by: hosting meetings, providing in-kind support to smaller community partners, creating a space for conversations around health equity and the importance of the SDOH. [...] This proposal for the MHN fit the issues and needs and the partners were ready to move on to the proposal. [...] The OLIP table thought it would be successful, SWCHC was able to support a learning trip to Edmonton and then support the development of the proposal and the Champlain LHIN recognized the issue and agreed to fund the pilot project for a year.
